我是直接写信去给 CBP 在 LAX
CBP 回复时附上两个 Forms , 还有其他需要的文件，
：那两个 Forms 要印出来签名，不可以用电子签名

I94_Affidavit
For a complete applicatoin, you will need:
(1) passport biographical page, (2) original departure flight itinerary, (3)
proof that airline cancelled flight due to COVID-19 related issues (Travel
ban, airport closures, decreased flight operation, etc.) and (4) booking
confirmation for future departure (Future travel must be the next “available
flight” out of the United States) (5) satisfactory departure request form
, and (6) Affidavit for DHS (both filled and signed).
我收到国泰航空寄来的不能在香港转机的通知，国泰航空帮我
改定到五月底，
附上那封信跟后来的订位纪录，两天后就收到 CBP 的通知说那个 I94 延长三十天
