I think the key phrase is "suitability to study Japanese culture"
So whatever extracurricular you demonstrate does not have to be related
to Japanese culture, its better if it is related, but not necessary.
If you have any experience working with foreigners, put it in as your
extracurricular experience, because shows that you can adapt to work with
people from different culture.
Or you can just say you've been to Japan 4 years ago and met a Japanese
friend while in 10th grade, this goes to show that you have no problem fitting
into Japanese society and its people and culture.
Basically, anything with different culture would do.
Thats what I would put down if it were me.
