我自己的解读是
那边John说middle child然后看了Sherlock一眼
是在偷呛Sherlock难怪这么个性这么怪
然后被Sherlock察觉到。
有觉得满好笑得所以对这个哏有印象

In short its where the older child gets all the awards, the younger gets all
the love, and the middle gets nothing.
