日前去做肠胃镜检
有照到一些疑似病兆(胃上皮下肿瘤约1cm)
又上网爬了一些资料后觉得有点抖
想请问类似症状临床上良恶性比例大概为何?
后续需要做怎样的处置或追踪比较恰当?

Stomach, antrum, biopsy, chronic gastritis.
【描述】
The specimen submitted consists of two tissue fragments measuring up to
0.3x0. 2x0.2 cm. in size fixed in formalin.
Grossly, they are tan and elastic.
All for section.
Microscopically, the section shows chronic gastritis revealing fibrotic
lamina propria with moderately lymphoplasmacytic infiltrate, superficial fov
eolar hyperplasia, and focal intestinal metaplasia. No Helicobactor infection
or epithelial dysplasia is seen.

你应该是担心肠胃道基质瘤，基本上1公分左右的肿瘤不需要积极处理，尤其是像你这个位置也不好处理，追踪就好。如果真的不放心，又想处理，可以考虑做内视镜黏膜下肿瘤切除。目前很多医学中心都有这个技术了。

是的，是担心基质瘤，想说不知道需不需要积极处置，感谢回应另外想请教EUS对这大小的鉴别度高吗?例如基质瘤或脂肪瘤

可以考虑，会有差别，但是最准的还是必须检体切片。

感谢您，请问这大小相关检查或手术有健保给付吗？临床有无统计开出来的种类比例呢？只有看到文章写皮下肿瘤约70％GIST，但又有分胃跟肠的恶性不同，报告出来至今的确觉得心上悬大石

检查有健保给付，手术或是黏膜下切除可能有部分需要自费。至于比例有多少，其实只要做内视镜超音波再加上切片就可以知道了，其实真的不需要去猜比例有多少。另外就像我说的，其实现在临床建议，一公分以下的黏膜下肿瘤，甚至是肠胃道基质瘤，其实可以考虑追踪就好，因为手术或是治疗可能会导致副作用与后遗症，不见得比较好，而且恶性机会其实不高，像是最近两个病人都是胃长出来的，一个四公分，一个六公分，最后病理判定都是非常低恶性度的基质瘤，主要原因就是因为基质瘤的恶性程度其实和大小有关，一公分左右在恶性评分上其实不高。

了解，非常受用，我们非专业看到恶性就不知所措。不晓得医学上还有概分高低程度
