想请问一下，关于附件C栏中的
"申请人签署的声明书，表明他/她打算在E身分结束时离开美国"
这个声明书有格式规定或范本吗？
还是只要短短的用英文写
"I intend to depart i intend to depart the country, U.S.A,
right after my E-2 VISA expired."
然后底下签名就好？
